Thank you for your interest in ServSafe Certificate training!

The first step is to complete the form below. Please note that our ServSafe Certificate program is funded by the City of Hartford’s Community Development Block Grant and requires participants to be:

  • Hartford residents seeking employment
  • Individuals employed in Hartford or employers operating in Hartford
  • U.S. work-authorized individuals
  • Entrepreneurs starting a business in Hartford
  • Businesses relocating a food service operation to Hartford

CDBG/ServSafe Training Forms & Affidavits

After you submit your Training Application, the Coordinator will explain which of the following forms you will need to complete:

  1. Self-Certification of Annual Income by Beneficiary Form (all trainees)
  2. CDBG Qualifying Criteria for Businesses Form
  3. CDBG Qualifying Business Affidavit
  4. HUD – CDBG Client Ethnic /Racial Definitions
